At the Wise Center for Plastic Surgery, we often receive patients who are concerned regarding the amount of stubborn fat they have in the chin and neck area. While many patients have enough skin laxity (looseness) in the neck to undergo treatments such as a facelift or neck lift, some patients dealing with submental fat (a double chin) simply don’t have enough excess skin to make those treatments feasible.

These patients are usually young and have a vibrant look but are struggling with what is often referred to as an "obtuse" neckline. This essentially means that the region beneath the chin and into the neck is devoid of contours and shape. Fortunately, Submentoplasty is able to address this issue by doing away with excess fat in the neck region.

What is Submentoplasty?

A submentoplasty is a surgical procedure that is designed to permanently improve the shape and contours of the neckline. The procedure works by removing fatty deposits from the neck and tightening the underlying muscles in both the chin and neck regions.

What makes a submentoplasty procedure different from a neck lift is that it is far less invasive and focuses specifically on the areas of the chin and the upper neck. Instead of removing excess skin from the area, a submentoplasty procedure focuses on removing excess fat deposits underneath the chin that do not respond to weight loss.

Who is an ideal candidate for Submentoplasty?

Submentoplasty is a great option for patients who are dealing with a ‘double chin’ or any amount of excess fat in the neck region that they find to be bothersome. During the private consultation, the patient’s current status and medical history will be fully assessed to confirm their candidacy for Submentoplasty or recommend another procedure that is more suitable.

Patients who have the following conditions are considered to be ideal candidates for a Submentoplasty procedure:

  • Unwanted fat beneath the chin that is unaffected by diet and exercise
  • A non-contoured angle going from the chin down to the neck
  • Excessively tight skin in and around the neck area

Younger patients often have good skin elasticity but still suffer from excess fat beneath the chin, even if they are at a healthy weight. Submentoplasty can be employed to quickly and effectively treat it. While submentoplasty procedures do not affect the lower portion of the face, they can have a massive impact on the face and reverse signs of aging. What is the procedure like?

During a Submentoplasty procedure, small incisions are made underneath the chin. Then, the central neck muscles are tightened, and the vertical bands in the neck are released. This creates a better-defined angle in the neck (often referred to as the cervico-mental angle). During this time, liposuction may also be performed to further improve the contours of the neck.

A submentoplasty procedure can be carried out in as little as 45 minutes and comes with less downtime and a shorter recovery period than a neck lift. As the treatment involves shorter incisions, there will be fewer sutures and easier aftercare instructions (plus fewer scars). That said, patients are likely to experience some swelling in the treatment area for anywhere from one to two weeks following their procedure.

What is the recovery process like?

Following the procedure, the patient’s neck is fitted with a snug dressing which they must keep on for three days. Bruising and swelling may last for one to two weeks. Sutures are usually removed at the one-week mark. During the first three to four weeks of the recovery process, it is normal for patients to experience tightness and itching in the treatment areas.
